Steps of add help file in App ???

hi,
I want to know that whats the steps of adding help file in App?
Suppose I have an App "XYZ" , there is a menu Help in options menu. I want to show a help file regarding App when user click on Help menu. so whats the process of this ?
and also one more thing that there are a menu "About" in several Application in which when user click there display information regarding app and company in brief ways... how I do this in my App?? Plz help me ......
